HEAL clinic appointments

For ages 2 to 18
First visits are held on: 
• Tuesdays, 9 a.m. to 1 p.m. or
• Fridays, 1 to 5 p.m.
Follow-up visits are held on:
• Wednesdays, 1 to 4 p.m.

HEAL Program
What happens during the six office visits?

  • Visit 1: Initial Evaluation and Assessment
    A healthcare professional builds a relationship with the patient
    and family. The visit includes a thorough physical exam and
    establishes program goals.
  • Visit 2: Nutrition
    A registered dietitian discusses healthy eating choices, including
    current food options and offers an individualized nutrition plan.
  • Visit 3: Physical Activity
    An exercise specialist discusses active living habits and offers
    exercise prescriptions.
  • Visit 4: Feelings and Families
    A mental health specialist discusses body image and self-esteem,
    as well as parenting, family dynamics and cultural issues.
  • Visit 5: Healthy Lifestyle
    Families discuss with the dietitian or exercise specialist their
    progress in self-managing their goals or how to modify them as
    needed.
  • Visit 6: Summary Visit
    The team and family assess accomplishments and progress toward goals, and develop a plan for sustaining healthy eating and active living habits.

Follow-up visits

  • Follow-up visits are scheduled at three and
    six months.


Other weight management services:

Kids Gettin' Fit!

• Ages 6-12, with a parent in attendance
• A six-week program taught by a registered dietitian
• A weekly two-hour class offered on a weekday, once in the winter, summer, spring and fall

Teen Talks
• Eight-week program, one-hour sessions on Tuesday afternoons
• Multidisciplinary program with physician, nurse practitioner, registered dietitian, exercise physiologist and psychologist

Fit Families Class
• Class designed for early anticipatory guidance ages 6 to 12 (all ages are welcome)
• A two-hour class taught by registered dietitian
• Offered once a month on Wednesday from 4-6pm
